Покажите sh flash:
там должен быть mwr1941-iprank9-mz.124-16.MR.bin
но, видимо, его там нет и рутер грузится с onboard ROM.
Решение: залить правильный mwr1941-iprank9-mz.124-16.MR.bin на flash:

Всё верно. Ещё варианты:
1. Не совсем правильно boot system flash mwr1941-iprank9-mz.124-16.MR.bin
Правильно: boot system flash:mwr1941-iprank9-mz.124-16.MR.bin
2. MWR1841 может работать в режиме standalone и redundancy, в последнем случае они как-то хитро грузится, надо читать.

т.к. наблюдалось

When you get the above error messages on the console output, it means the Flash is empty or the file system is corrupted. Copy a valid image on the Flash by following the procedures provided in this document.
Т.е. рекомендуют format flash: и заново залить ios.
И можно удалить все boot system из конфига

все сделал так же загружается после второй попытки.

Это не совсем ошибки. Скорее предупреждения IP-RAN (IP Radio Access Network) о том, что часть интерфейсов недоконфигурированы.
