Есть вот такой прибор, 0aec:3260 Neodio Technologies Corp. 7-in-1 Card Reader.
Была эта бага года 3-4 назад, как я её победил напрочь не помню :)

Помню, надо было
1. ehci_usb грузить раньше uhci_hcd
2. echo 8 >  /sys/module/scsi_mod/parameters/max_luns;

MAX_LUNS сейчас и так 512, по дефолту.
